ALS Case Managers – Your Point of Support After an ALS Diagnosis!

ALS is a condition that, from the very beginning, involves a significant amount of administration, practical support, and medical care—often in an intensive and recurring manner.
Despite all these challenges, most patients express the wish to remain at home until the very end. At the ALS League, we are fully committed to fulfilling this wish—or any other that contributes to the quality of life of people living with ALS (pALS).
Proper guidance is therefore essential and can greatly strengthen the resilience of both pALS and their loved ones.
Recognizing the need for professional support focused on quality of life, the federal government launched a pilot project on case management as part of integrated care. The ALS League now employs both a Dutch-speaking and a French-speaking case manager.
The goal of the case managers is to help pALS identify and address their needs and concerns while designing and following up on their multidisciplinary care path.
The case manager is a trusted figure—not a care provider themselves, but someone who stands closely alongside the patient. They act as a liaison between the patient, their loved ones, and the various care providers. They coordinate, collaborate, connect, advise, and support across all areas of life (living, housing, care, etc.) until the pALS and their support network feel empowered to navigate the illness more independently. The case manager also monitors the care and support plan.
Patients, their loved ones, and care providers can access the support of a case manager free of charge at any stage of the illness.
